Harp/Bench Rental Form - Due May 1

Limited rental harps are available for students who are flying to the seminar and cannot bring their own harp.

Students renting harps will have access to a shared pool of harps for practice and performance. This system is designed to conserve practice room space and ease harp moving to ensembles and classes. This shared pool of harps will have a variety of styles and sizes, mostly concert grands. Students renting harps are required to provide their own tuning key.

Airport Shuttle Form - Due May 1

Please complete this form if you require airport shuttle service.

Arrival flights should be scheduled to arrive before 11 am on July 7, 2024.

Departing flights should be scheduled after 12 pm on July 15, 2024.

Please do not book flights until after they are approved by Susan Brady.

2024 Prep Shuttle Request Form

PREP Airport Shuttle Services

Payment information for the shuttle will be available after your flight times are approved.
Must arrive before 11:00 AM on July 7 and depart after 2:00 PM on July 15th.
For Arrival: Includes gate pickup within 30 minutes of original scheduled flight time. For delayed flights arriving later than the original 30 minutes, gate pick up will be an additional $20 an hour. For Departure: Includes gate drop off by 2:00 PM. For delayed flights and those leaving after 2:00 PM, gate drop off will be an additional $20 an hour. You will be billed separately, if either or both overtimes occur.
All returning flights are required to stay the extra night on July 14 since the shuttle does not leave until the morning of July 15. No meals are included, but there will be access to fast food.

MDH Camper Health History

This is a general health history form.

MGH Medication Administration Authorization – Personal Medication

This form is required submit if  you will be bringing any medications to YAHS. This includes any over-the-counter or prescription medications.

Please read and complete this form carefully. Please also note that all medications at YAHS are required by the Maryland Department of Health to include a physician signature.

Please upload all your completed paperwork to this form and submit by May 15, 2024.

If you require and will be bringing ANY kind of medication to YAHS, including any over the counter medications or vitamins (headache medicine included) you will need to download and print the Medication Administration Authorization PDF attached to the Student Information Center, fill it out, scan the completed form, and upload it here. (Also, If you don't plan on bringing any normal over the counter medication like headache medicine, but anticipate that you MAY require some during YAHS, you need to fill out this form as well)

This form is important in ensuring our faculty are prepared to help you learn at the seminar. If you need to make a change after you submit this form please email info@harpseminar.com with your changes.
